This Purchase Order authorizes the engagement of a qualified Physiotherapist to deliver comprehensive rehabilitation and therapeutic services at the Coastal Health & Rehabilitation Centre in Tanzania Dar es Salaam. 4.1 Scope of Work: The Physiotherapist shall provide all services listed above at the designated facility in Tanzania Dar es Salaam between the hours of 07:00 and 19:00, Monday through Saturday. All clinical interventions must comply with the Tanzania Physiotherapy Council practice guidelines and the National Health Policy of the United Republic of Tanzania.
4.2 Service Period: This Purchase Order covers a service period of six (6) months, commencing on 1 July 2025 and concluding on 31 December 2025, unless extended by mutual written agreement.
4.3 Payment Schedule: Payment shall be made in two (2) equal installments of TZS 54,221,000 each, due within thirty (30) calendar days of the end of each quarter. Invoices must reference this Purchase Order number (PO-TZ-2025-04782) and be submitted to the procurement department in Tanzania Dar es Salaam.
4.4 Professional Standards: The Physiotherapist must maintain valid professional registration with the Tanzania Physiotherapy Council throughout the contract period and carry professional indemnity insurance with a minimum cover of TZS 500,000,000.
4.5 Confidentiality: All patient records, clinical data, and operational information encountered during the provision of services in Tanzania Dar es Salaam shall be treated as strictly confidential in accordance with the Tanzania Data Protection Act, 2022.
4.6 Termination: Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination, the Physiotherapist shall be compensated for all services rendered up to the effective date of termination.
4.7 Dispute Resolution: Any disputes arising from this Purchase Order shall be resolved through mediation in Tanzania Dar es Salaam, and failing that, through the competent courts of the United Republic of Tanzania.
4.8 Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the United Republic of Tanzania.
