- This Purchase Order constitutes a formal request for Physiotherapist professional services to be delivered at the designated clinical facilities within United Kingdom Birmingham. All services must be performed by HCPC-registered Physiotherapists holding valid professional indemnity insurance.
- The Supplier shall ensure that every Physiotherapist assigned to this contract meets the minimum qualification of a BSc (Hons) in Physiotherapy and holds current registration with the Health and Care Professions Council (HCPC) of the United Kingdom.
- Service delivery shall commence no later than 1 August 2025 and continue for a period of twelve (12) months, subject to mutual renewal. The Physiotherapist team must maintain a minimum of 95% session attendance rate across all United Kingdom Birmingham sites.
- Payment shall be made within thirty (30) calendar days of receipt of a valid invoice. Invoices must reference this Purchase Order number (PO-BHM-2025-04782) and include the Supplier's VAT registration number.
- The Supplier is responsible for providing all clinical equipment, therapeutic modalities, and consumables required for the Physiotherapist sessions. Any additional equipment beyond the standard provision must be approved in writing by the Buyer prior to procurement.
- All patient data processed during Physiotherapist consultations must comply with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018. Data shall be stored exclusively on servers located within the United Kingdom.
- The Buyer reserves the right to audit service delivery, Physiotherapist qualifications, and clinical outcomes on a quarterly basis. The Supplier shall provide full access to relevant records within five (5) working days of a written request.
- Any variation to the scope of Physiotherapist services, including changes to session frequency, patient caseload, or delivery location within United Kingdom Birmingham, must be documented via a formal Purchase Order amendment signed by both parties.
- Termination may be issued by either party with ninety (90) days' written notice. In the event of termination, the Supplier shall complete all in-progress Physiotherapist treatment plans and transfer patient records to the Buyer or a nominated successor within fourteen (14) days.
- This Purchase Order is governed by the laws of England and Wales. Any disputes arising from the provision of Physiotherapist services under this agreement shall be subject to the exclusive jurisdiction of the courts of the United Kingdom Birmingham.
