1. This Purchase Order shall become effective upon written acceptance by the designated Psychiatrist and countersignature by the Procurement Officer of the Ministry of Public Health, Afghanistan Kabul.
2. The Psychiatrist shall commence duties no later than 01 August 2025 at the Central Mental Health Facility, Kabul, Afghanistan. Failure to report by this date shall constitute a material breach of this Purchase Order.
3. The Psychiatrist is required to work a minimum of five (5) days per week, with a standard working schedule of 08:00 to 16:00, subject to emergency call-out requirements as determined by the facility administration in Afghanistan Kabul.
4. All patient records, clinical notes, and treatment documentation generated under this Purchase Order shall be maintained in accordance with the data protection regulations of Afghanistan and shall remain the property of the Ministry of Public Health, Afghanistan Kabul.
5. The Psychiatrist shall comply with all ethical standards of psychiatric practice as outlined by the World Health Organization (WHO) and the local medical council of Afghanistan.
6. Payment shall be made on a monthly basis within thirty (30) days of receipt of the monthly clinical report and a valid invoice submitted to the accounts department in Afghanistan Kabul.
7. This Purchase Order may be terminated by either party with a written notice of sixty (60) days. In the event of termination, the Psychiatrist shall ensure a smooth transition of all ongoing patient care to a successor professional.
8. The Psychiatrist shall maintain valid professional liability insurance with a minimum coverage of USD 500,000 throughout the duration of this Purchase Order.
9. All disputes arising from this Purchase Order shall be resolved through arbitration in Kabul, Afghanistan, in accordance with the commercial arbitration laws of the Islamic Republic of Afghanistan.
10. The Psychiatrist shall not subcontract or delegate any portion of the services described in this Purchase Order without prior written consent from the Ministry of Public Health, Afghanistan Kabul.
