- This Purchase Order is issued by the Córdoba Provincial Health Services Administration for the procurement of Psychiatrist professional services to be rendered within the jurisdiction of Argentina Córdoba. The Psychiatrist named herein must hold a valid, active license issued by the Colegio de Médicos de Córdoba and must maintain professional liability insurance with a minimum coverage of ARS 50,000,000 for the entire contract duration.
- All Psychiatrist services described in Section 2 must be performed at the designated clinical facilities in Argentina Córdoba or at approved satellite locations as directed by the Mental Health Division. The Psychiatrist shall comply with all clinical protocols established by the provincial health authority and the national Ministry of Health of Argentina.
- Payment terms: Net 30 days from the date of invoice submission. Invoices must reference this Purchase Order number (PO-2025-CBA-04872) and include the Psychiatrist's CUIT, a detailed breakdown of sessions rendered, and corresponding patient file references (de-identified per Argentine data protection law 25.326). Payment shall be made via bank transfer to the account specified by the Psychiatrist.
- The Psychiatrist is required to submit monthly service reports to the Argentina Córdoba Mental Health Division by the 5th business day of the following month. Reports must include the number of sessions completed, patient outcomes summary, medication adjustments made, and any adverse events encountered during the provision of psychiatric care.
- This Purchase Order is valid for a period of six (6) months from the Required Service Date. Renewal is subject to mutual written agreement and satisfactory performance evaluation by the Argentina Córdoba health authority. Either party may terminate this Purchase Order with thirty (30) days written notice.
