- This Purchase Order is issued in accordance with the Australian Consumer Law (2010) and the Victorian Health Services Act 1988. All psychiatric services rendered under this agreement must comply with the standards set by the Australian Health Practitioner Regulation Agency (AHPRA).
- The Psychiatrist named in this document, Dr. Eleanor M. Whitfield, holds an active registration as a medical practitioner and a Fellow of the Royal Australian and New Zealand College of Psychiatrists (FRANZCP). The Psychiatrist shall maintain all required professional indemnity insurance with a minimum coverage of AUD $10,000,000 throughout the term of this Purchase Order.
- All services shall be delivered at the practice premises located in Australia Melbourne, specifically at 88 Swanston Street, Melbourne VIC 3000, or at such other location within the Melbourne metropolitan area as mutually agreed in writing by both parties.
- Payment shall be made within thirty (30) calendar days of receipt of a valid tax invoice. Invoices must reference this Purchase Order number (PO-2025-MEL-04872) and include the supplier's ABN for compliance with Australian Taxation Office requirements.
- The Psychiatrist shall maintain strict confidentiality of all patient records in accordance with the Privacy Act 1988 (Cth) and the National Privacy Principles. Clinical records shall be stored in compliance with Victorian health information legislation.
- Any controlled substances prescribed by the Psychiatrist must be documented in accordance with the Poisons Standard (Schedule 2 of the Therapeutic Goods Act 1989) and Victorian state regulations governing the dispensing of controlled medicines.
- This Purchase Order may be terminated by either party with fourteen (14) days written notice. In the event of termination, the Psychiatrist shall provide a minimum of two (2) transition consultations to ensure continuity of care for all patients under active management.
- All disputes arising from this Purchase Order shall be resolved in accordance with the Commercial Arbitration Act 2011 (Vic) and shall be subject to the exclusive jurisdiction of the courts of the State of Victoria, Australia Melbourne.
- The Psychiatrist warrants that all services provided under this Purchase Order shall be performed with due care, skill, and in accordance with the prevailing clinical guidelines published by the Royal Australian and New Zealand College of Psychiatrists.
This Purchase Order is authorised for release and the above terms are accepted by the undersigned parties. By signing below, both parties acknowledge that this document constitutes a binding agreement for the provision of Psychiatrist services within Australia Melbourne for the duration specified herein.
