Terms and Conditions of this Purchase Order
- This Purchase Order is issued by Tehran Metropolitan Health & Wellness Center and constitutes a binding agreement for the provision of Psychiatrist services within the city of Tehran, Iran, for the period specified above.
- The Psychiatrist, Dr. Saeed Mohammadi, shall maintain a valid medical license issued by the Iranian Ministry of Health and Medical Education throughout the duration of this Purchase Order. Any lapse in licensure shall be reported within 48 hours and may result in immediate termination.
- All psychiatric services shall be rendered in compliance with the regulations of the Iranian Medical Council and applicable mental health legislation of the Islamic Republic of Iran. Patient confidentiality shall be maintained in accordance with Iranian data protection standards.
- The Psychiatrist shall provide all services at the designated locations in Tehran, Iran, as outlined in the line items above. Relocation of service delivery sites requires prior written approval from the Purchasing Entity.
- Payment shall be processed monthly via the Shetab banking network. Invoices must be submitted by the 5th of each month for services rendered in the preceding month. Late payments beyond 15 business days shall incur a penalty of 0.5% per day.
- This Purchase Order may be terminated by either party with 30 days' written notice. In the event of termination, all services rendered up to the termination date shall be compensated in full.
- The Psychiatrist shall maintain professional liability insurance with a minimum coverage of 500,000,000 IRR, valid throughout the contract period in Tehran, Iran.
- All clinical records, patient files, and psychiatric reports generated under this Purchase Order shall be stored at the Tehran Metropolitan Health & Wellness Center facility and remain the property of the Purchasing Entity.
- Disputes arising from this Purchase Order shall be resolved through the competent courts of Tehran, Iran, in accordance with the Civil Procedure Code of the Islamic Republic of Iran.
- This Purchase Order is subject to the general procurement regulations of the Iranian government and any applicable tax obligations under the Iranian tax code.
