- This Purchase Order is valid for a period of twelve (12) months from the date of issue, covering the period from 01 July 2025 through 30 June 2026. The Psychiatrist shall render all services at the Al-Nakheel Medical Center facility located in Kuwait Kuwait City, or at such other approved location as mutually agreed upon in writing.
- The Psychiatrist must maintain a valid medical license issued by the Kuwait Ministry of Health and shall provide proof of current licensure, malpractice insurance coverage (minimum KWD 500,000), and professional indemnity insurance prior to commencing any services under this Purchase Order.
- All psychiatric services shall be delivered in accordance with the Kuwaiti Medical Practice Law, the Mental Health Protection Regulations, and the ethical standards established by the Kuwait Medical Association. Patient confidentiality shall be maintained in strict compliance with the data protection provisions applicable in Kuwait Kuwait City.
- Payment shall be made within thirty (30) calendar days of receipt of a valid invoice from the Psychiatrist. Invoices must reference this Purchase Order number (PO-2025-KWT-04782) and shall be submitted to the Accounts Payable Department of Al-Nakheel Medical Group LLC. Payment shall be processed via bank transfer to the account designated by the Psychiatrist.
- The Psychiatrist is required to submit monthly service reports detailing the number of sessions conducted, patient outcomes, medication adjustments, and any adverse events. Failure to submit reports for two consecutive months may result in suspension of this Purchase Order pending review.
- Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination, all services rendered up to the effective date of termination shall be invoiced and paid in full. No liability shall attach to either party for services not yet rendered.
- All disputes arising from this Purchase Order shall be resolved through amicable negotiation. Should negotiation fail, the matter shall be referred to the competent courts in Kuwait Kuwait City in accordance with the laws of the State of Kuwait.
- This Purchase Order does not constitute an employment relationship. The Psychiatrist is engaged as an independent contractor and shall be responsible for all personal taxes, social insurance contributions, and professional obligations as required by Kuwaiti law.
- Al-Nakheel Medical Group LLC reserves the right to audit service records, patient files (in anonymized form), and financial documentation related to this Purchase Order upon reasonable notice of no less than five (5) business days.
