This Purchase Order is issued by Meridian Health & Wellness Pte. Ltd. to formally engage the services of a qualified Psychiatrist to provide comprehensive psychiatric care, consultation, and therapeutic services to our corporate clients and their dependents within Singapore Singapore. The Psychiatrist shall be responsible for the following duties and deliverables as outlined in this Purchase Order:

The Psychiatrist engaged under this Purchase Order must meet the following mandatory qualifications and professional standards as recognised in Singapore Singapore:

• Full registration with the Singapore Medical Council (SMC) and valid Specialist Registration in Psychiatry.
• Minimum of eight (8) years of post-qualification clinical experience in adult and occupational psychiatry.
• Membership of the Singapore Academy of Medicine (SAM) and the Singapore Psychiatric Association.
• Valid professional indemnity insurance coverage of no less than SGD 2,000,000 per claim.
• Demonstrated experience in corporate and occupational mental health settings within Singapore Singapore.
• Proficiency in English and Mandarin, with working knowledge of Malay or Tamil preferred for diverse client populations.
• Compliance with all applicable regulations under the Mental Health (Care and Treatment) Act of Singapore.

• Confidentiality: The Psychiatrist shall maintain strict confidentiality of all patient records and clinical information in accordance with the Personal Data Protection Act (PDPA) of Singapore Singapore. No patient data shall be disclosed to third parties without written consent.
• Service Standards: All psychiatric consultations and services shall be delivered in a timely manner. Initial assessments must be scheduled within five (5) business days of referral. Follow-up appointments must be arranged within ten (10) business days.
• Reporting: The Psychiatrist shall submit a monthly summary report to the Purchasing Entity detailing the number of sessions conducted, general trends in mental health referrals, and any critical incidents requiring management attention. Reports shall be submitted by the 5th business day of the following month.
• Termination: Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination, all outstanding invoices for services rendered up to the termination date shall be settled in full.
• Dispute Resolution: Any disputes arising from this Purchase Order shall be resolved through mediation in Singapore Singapore, and failing mediation, through arbitration under the rules of the Singapore International Arbitration Centre (SIAC).
• Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the Republic of Singapore.
• Reimbursement: Travel expenses incurred by the Psychiatrist for on-site visits to client locations within Singapore Singapore shall be reimbursed at actual cost, subject to prior written approval and submission of valid receipts.
