This Purchase Order authorises the engagement of a qualified Psychiatrist to provide comprehensive psychiatric assessment, diagnosis, treatment planning, and ongoing clinical management services at the Cape Town Regional Psychiatric Hospital and affiliated community health centres in the greater Cape Town, South Africa metropolitan area. The Psychiatrist shall operate in full compliance with the Mental Health Care Act 17 of 2002 (South Africa), the National Health Act 61 of 2003, and all applicable regulations of the Health Professions Council of South Africa (HPCSA).

All amounts are quoted in South African Rand (ZAR). VAT is applicable at the current South African rate of 15% as per the Value-Added Tax Act 89 of 1991.

  1. Professional Compliance: The Psychiatrist shall maintain an active and unrestricted registration with the Health Professions Council of South Africa (HPCSA) for the entire duration of this Purchase Order. Any lapse in registration shall constitute grounds for immediate termination.
  2. Confidentiality & POPIA: All patient information shall be handled in strict accordance with the Protection of Personal Information Act 4 of 2013 (POPIA) of South Africa. The Psychiatrist shall not disclose any patient records to third parties without written consent or a lawful court order.
  3. Service Delivery Location: All services under this Purchase Order shall be rendered at the designated facilities within Cape Town, South Africa, as specified in Section 2. Off-site community outreach shall be limited to the City of Cape Town municipal boundaries.
  4. Medication Prescribing: The Psychiatrist shall prescribe psychotropic and controlled substances in full compliance with the Medicines and Related Substances Act 101 of 1965 and its amendments. All Schedule 5 and 6 prescriptions shall be issued on the prescribed South African government forms.
  5. Reporting: The Psychiatrist shall submit monthly service delivery reports to the Department of Mental Health & Substance Abuse Services by the 5th business day of the following month. Reports shall include patient volumes, treatment outcomes, medication utilisation, and any adverse events.
  6. Payment: Payment shall be made within thirty (30) calendar days of receipt of a valid tax invoice referencing this Purchase Order number (PO-CT-2025-04871). Late payment shall attract interest at the rate prescribed by the Late Payment of Interest to Suppliers Act 15 of 2011 (South Africa).
  7. Termination: Either party may terminate this Purchase Order with thirty (30) days written notice. The issuing entity may terminate immediately in the event of professional misconduct, HPCSA disciplinary action, or breach of POPIA obligations.
  8. Governing Law: This Purchase Order shall be governed by and interpreted in accordance with the laws of the Republic of South Africa. Any disputes shall be resolved through the Cape Town District Court or, where applicable, the South African Labour and Employment Court.
  9. Insurance: The Psychiatrist shall maintain professional indemnity insurance with a minimum cover of ZAR 5 000 000 per claim, issued by a South African-registered insurer, for the duration of this engagement.

This Purchase Order is issued and authorised by the undersigned on behalf of the Cape Town Metropolitan Health Authority. The Psychiatrist acknowledges acceptance of all terms, conditions, and service requirements outlined herein by signing below.
