- This Purchase Order constitutes a formal request for Psychiatrist professional services to be delivered within the United Kingdom Birmingham metropolitan area. All services shall be rendered in accordance with the General Medical Council (GMC) Good Medical Practice guidelines and the NHS England standards for mental health provision.
- The Psychiatrist named in this Purchase Order shall maintain full professional indemnity insurance with a minimum cover of £5,000,000 per claim throughout the duration of the contract. Evidence of insurance shall be provided prior to the commencement of services.
- All psychiatric consultations, assessments, and reports ordered under this Purchase Order shall be conducted at the designated service location in Birmingham, United Kingdom, or at such alternative venue within the Birmingham postcode area as mutually agreed in writing by both parties.
- Payment shall be made within thirty (30) calendar days of receipt of a valid VAT invoice referencing this Purchase Order number (PO-BHM-2025-04872). Invoices must be submitted to the Birmingham Metropolitan Health & Wellbeing Trust Accounts Payable department at the address stated above.
- The Psychiatrist shall comply with all applicable data protection legislation, including the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018. Patient records and clinical documentation generated under this Purchase Order shall be stored in accordance with NHS Information Governance standards.
- Any variation to the scope, quantity, or rate of Psychiatrist services outlined in this Purchase Order must be authorised in writing by the undersigned procurement officer prior to execution. Verbal amendments shall not be binding.
- This Purchase Order is governed by the laws of England and Wales. Any disputes arising from the interpretation or performance of this Purchase Order shall be subject to the exclusive jurisdiction of the courts of Birmingham, United Kingdom.
- The Psychiatrist shall provide a monthly service activity report detailing sessions delivered, patient outcomes, and any clinical incidents to the Birmingham Metropolitan Health & Wellbeing Trust within five (5) working days of the end of each calendar month.
- Either party may terminate this Purchase Order arrangement with thirty (30) days' written notice. Upon termination, all outstanding Psychiatrist services already scheduled shall be completed, and final settlement shall be made in accordance with the payment terms herein.
