- Professional Licensure: The Psychologist named in this Purchase Order must maintain an active registration with the British Columbia College of Psychologists (BCCP) for the entire duration of the engagement. Any lapse in licensure shall constitute grounds for immediate termination of this Purchase Order without penalty to the Buyer.
- Confidentiality and Privacy: All client records, session notes, and diagnostic data shall be handled in strict compliance with the Personal Information Protection Act (PIPA) of British Columbia and the Privacy Act of Canada. The Psychologist shall not disclose any identifiable information to the Buyer beyond aggregate, de-identified wellness metrics unless written consent is obtained from the client.
- Service Delivery Location: All in-person sessions shall be conducted at the Psychologist's registered office in Canada Vancouver or at the Buyer's premises located at 1200 West Georgia Street, Canada Vancouver. Telehealth sessions shall be delivered via a HIPAA/PIPA-compliant encrypted platform.
- Payment Schedule: Invoices shall be submitted monthly on the last business day of each calendar month. Payment is due within thirty (30) days of invoice receipt. Late payments shall accrue interest at 1.5% per month as permitted under the Interest Act of Canada.
- Scope Limitations: This Purchase Order covers psychological services only. The Psychologist shall not provide medical diagnosis, prescribe medication, or perform any procedure outside the scope of practice defined by the BCCP. Referrals to psychiatrists or other medical professionals may be made at no additional cost to the Buyer.
- Termination: Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination, the Buyer shall be liable only for services rendered up to the effective date of termination.
- Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the Province of British Columbia and the federal laws of Canada applicable therein. Any disputes shall be resolved through mediation in Canada Vancouver before proceeding to litigation in the Supreme Court of British Columbia.
- Insurance: The Psychologist shall maintain professional liability insurance with a minimum coverage of $5,000,000 CAD and shall provide a certificate of insurance upon request.
- Compliance: The Psychologist shall comply with all applicable federal and provincial regulations, including but not limited to the Health Professions Act (BC), the Occupational Health and Safety Regulation (BC), and the Canadian Human Rights Act.
This Purchase Order is effective upon signature by both parties. By signing below, the Buyer authorizes the expenditure of funds up to the total Purchase Order value stated above, and the Psychologist accepts the terms, conditions, and scope of services outlined in this document.
