1. Service Location: All in-person psychological services under this Purchase Order shall be conducted at the Tokyo Mind & Body Psychological Clinic, 5F Marunouchi Building, Chuo-ku, Tokyo, Japan, or at the Sakura Wellness Group headquarters in Shibuya-ku, Tokyo, Japan, as mutually agreed in writing.
2. Professional Standards: The Psychologist and all clinical staff shall adhere to the Ethical Code of the Japanese Association of Clinical Psychology, the National Board of Examiners in Psychology (Japan), and all applicable regulations of the Ministry of Health, Labour and Welfare, Japan.
3. Confidentiality: All psychological records, session notes, and assessment data pertaining to Sakura Wellness Group employees shall be treated as strictly confidential in compliance with the Act on the Protection of Personal Information (APPI) of Japan.
