- This Purchase Order constitutes a binding agreement between Al-Nahda Healthcare Group, WLL (hereinafter "the Buyer") and Dr. Layla Al-Mubarak, Licensed Clinical Psychologist (hereinafter "the Service Provider"), for the provision of psychological and mental health services as described herein.
- All services shall be rendered at the Service Provider's registered practice located in Kuwait Kuwait City, or at such alternative location within Kuwait Kuwait City as mutually agreed upon in writing by both parties. The Psychologist shall ensure that all facilities comply with the standards set forth by the Kuwait Public Health Authority (KPHC).
- The Psychologist shall maintain full professional confidentiality in accordance with Kuwaiti data protection regulations and the ethical code of the Kuwait Psychological Society. All client records shall be stored securely and shall not be disclosed to any third party without written consent, except where required by law.
- Payment shall be made within thirty (30) calendar days from the date of receipt of a valid invoice. Invoices shall be submitted monthly and must reference this Purchase Order number (PO-2025-KW-0347). Payment shall be made via bank transfer to the account designated by the Service Provider.
- The Service Provider warrants that she holds a valid and current license to practice as a Psychologist in the State of Kuwait and that all services shall be delivered in accordance with the highest professional standards recognized in Kuwait Kuwait City.
- Either party may terminate this Purchase Order by providing thirty (30) days' written notice. In the event of early termination, the Buyer shall be liable for services already rendered up to the date of termination.
- Any disputes arising from this Purchase Order shall be resolved through amicable negotiation. Failing resolution within thirty (30) days, the matter shall be referred to the competent courts in Kuwait Kuwait City in accordance with the laws of the State of Kuwait.
- The Service Provider shall carry professional indemnity insurance with a minimum coverage of KWD 100,000, valid for the duration of this Purchase Order. A certificate of insurance shall be provided upon request.
- All services are subject to the availability of the Psychologist. In the event of unavailability, the Service Provider shall arrange a qualified substitute Psychologist of equivalent credentials, subject to the Buyer's prior written approval.
- This Purchase Order is governed by and shall be construed in accordance with the laws of the State of Kuwait. The venue for any legal proceedings shall be Kuwait Kuwait City.
