Terms and Conditions of this Purchase Order
- This Purchase Order is issued by Meridian Corporate Wellness Sdn. Bhd. for the procurement of professional Psychologist services to be delivered in Malaysia Kuala Lumpur. The Psychologist engaged must hold a valid registration with the Malaysian Psychological Society (MPS) and possess a current practising licence issued by the relevant Malaysian regulatory authority.
- All Psychologist services described in this Purchase Order shall be performed at the buyer's premises located in Malaysia Kuala Lumpur, unless otherwise agreed in writing by both parties. The Psychologist is expected to be physically present in Malaysia Kuala Lumpur for all scheduled sessions and workshops.
- Payment for this Purchase Order shall be made within thirty (30) calendar days from the date of receipt of a valid tax invoice from the Psychologist's registered company. Payment shall be made via bank transfer to the account specified on the invoice.
- The Psychologist shall maintain strict confidentiality in accordance with the Malaysian Personal Data Protection Act 2010 (PDPA) and the professional ethical code of conduct of the Malaysian Psychological Society. All employee psychological records shall be stored securely in Malaysia Kuala Lumpur and shall not be disclosed to any third party without written consent.
- The Psychologist shall provide a minimum of forty-eight (48) hours' written notice for any cancellation or rescheduling of sessions. Cancellations made within forty-eight hours shall be subject to a 50% fee of the scheduled session rate.
- This Purchase Order is valid for a period of twelve (12) months from the date of acceptance. Any extension or amendment to the scope of Psychologist services must be documented in a written addendum signed by both parties in Malaysia Kuala Lumpur.
- In the event of a dispute arising from this Purchase Order, both parties agree to resolve the matter through mediation in Malaysia Kuala Lumpur before resorting to litigation in the courts of Malaysia.
- The Psychologist shall carry professional indemnity insurance with a minimum coverage of RM 1,000,000 for the duration of this Purchase Order and shall provide a certificate of insurance upon request.
- All deliverables, including written reports, assessment results, and workshop materials produced by the Psychologist under this Purchase Order, shall be the intellectual property of the purchasing party unless otherwise stated.
