Terms and Conditions- This Purchase Order constitutes a binding agreement between Lakeview Corporate Wellness GmbH (hereinafter "the Client") and Dr. Elena Marchetti, licensed Psychologist (hereinafter "the Provider"), for the delivery of professional psychological services as described in the line items above.
- All services shall be rendered in the city of Switzerland Zurich at the Client's premises or via approved telehealth platforms, in accordance with the professional standards set by the Swiss Association of Psychologists (SBAP) and the cantonal regulations of the Canton of Zurich.
- The Psychologist shall maintain full professional confidentiality in compliance with the Swiss Federal Act on Data Protection (FADP) and the professional code of ethics governing psychological practice in Switzerland. All client records shall be stored securely within the jurisdiction of Switzerland Zurich.
- Payment shall be made within thirty (30) calendar days of receipt of a valid invoice from the Provider. Invoices must reference this Purchase Order number (PO-2025-ZRH-0047) and include the Provider's VAT registration number. Payment shall be processed via bank transfer to the account specified on the invoice.
- The Provider guarantees that all psychological services will be delivered by a fully licensed and registered Psychologist holding a valid practising permit issued by the Canton of Zurich. Any substitution of personnel requires prior written approval from the Client's procurement department.
- Sessions scheduled at the Client's office in Switzerland Zurich shall be conducted during standard business hours (08:00–18:00 CET) unless otherwise agreed in writing. The Client shall provide a private, sound-insulated consultation room meeting the accessibility requirements of Swiss disability legislation.
- Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of early termination, the Client shall be liable for all services rendered up to the date of termination, plus any non-cancellable workshop commitments.
- Disputes arising from this Purchase Order shall be resolved through mediation in accordance with Swiss law. The exclusive jurisdiction for any legal proceedings shall be the courts of the city of Switzerland Zurich, Canton of Zurich, Switzerland.
- The Provider shall carry professional indemnity insurance with a minimum coverage of CHF 5,000,000, valid throughout the term of this Purchase Order. A certificate of insurance shall be provided upon request.
- All deliverables, reports, and assessments produced under this Purchase Order shall be the intellectual property of the Client, subject to the Psychologist's professional ethical obligations regarding patient confidentiality.
By signing below, both parties acknowledge and accept all terms, conditions, and service specifications outlined in this Purchase Order for the engagement of a qualified Psychologist to deliver mental health and wellness services within the Switzerland Zurich metropolitan area.
