- This Purchase Order is issued by the Kampala Regional Health Authority and constitutes a binding agreement for the provision of Psychologist services within Uganda Kampala. All terms are governed by the laws of the Republic of Uganda.
- The Psychologist named in this Purchase Order must maintain a valid professional license issued by the Uganda Psychologists Council throughout the entire contract period. Any lapse in licensure shall result in immediate suspension of this Purchase Order.
- Payment terms: Net 30 days from the date of invoice submission. Payment shall be made via bank transfer to the account specified by the Psychologist. The Psychologist must provide a valid Tax Invoice bearing the Uganda Revenue Authority (URA) invoice number.
- All services described in this Purchase Order shall be delivered at the designated facilities within Uganda Kampala unless otherwise agreed in writing by both parties. The Psychologist is responsible for all travel and transportation costs within Kampala city limits.
- The Psychologist shall adhere to the Code of Ethics of the Uganda Psychologists Council and all applicable data protection regulations under the Uganda Data Protection and Privacy Act, 2019. Patient confidentiality is paramount and must be maintained at all times.
- Any amendments, cancellations, or modifications to this Purchase Order must be made in writing and signed by both the authorized representative of the Kampala Regional Health Authority and the Psychologist.
- The Psychologist shall carry professional indemnity insurance with a minimum coverage of UGX 50,000,000 for the duration of this Purchase Order. A certificate of insurance must be provided prior to the commencement of services.
- Disputes arising from this Purchase Order shall be resolved through mediation in Uganda Kampala before either party may pursue litigation in the competent courts of Uganda.
- This Purchase Order is valid for a period of three (3) months from the date of issue, covering the service period from 1 July 2025 to 30 September 2025.
