This Purchase Order is issued by Al-Nakheel Medical Group to formally engage the services of a qualified Radiologist for the provision of diagnostic imaging interpretation, interventional radiology procedures, and related clinical support at the Al-Nakheel Imaging Centre located in Kuwait Kuwait City. The Radiologist shall perform all duties in accordance with the standards set forth by the Kuwait Ministry of Health and the Kuwait Medical Council.

1. This Purchase Order constitutes a binding agreement between the Purchasing Entity and the Radiologist upon signature by both parties. All services shall be rendered at the designated facility in Kuwait Kuwait City unless otherwise agreed in writing.
2. The Radiologist shall maintain a valid Kuwait Medical Council license and a current Ministry of Health practice registration throughout the duration of this Purchase Order. Any lapse in licensure shall constitute grounds for immediate termination.
3. All diagnostic reports, imaging interpretations, and interventional procedure documentation produced by the Radiologist shall be submitted to the Al-Nakheel Medical Group records department within Kuwait Kuwait City no later than 24 hours following the procedure or imaging session.
4. Payment for services rendered under this Purchase Order shall be made in Kuwaiti Dinars (KWD) via bank transfer to the account designated by the Radiologist. Invoices are due within thirty (30) calendar days of receipt.
5. The Radiologist shall comply with all data protection and patient confidentiality regulations as mandated by the Kuwait Ministry of Health and applicable Kuwaiti law. All patient records generated in Kuwait Kuwait City shall be stored in accordance with institutional policy.
6. Any modification, amendment, or extension of this Purchase Order must be documented in a written addendum signed by both parties. Verbal agreements regarding the scope of the Radiologist's duties in Kuwait Kuwait City shall not be enforceable.
7. In the event of a dispute arising from the interpretation or execution of this Purchase Order, both parties agree to first attempt resolution through mediation in Kuwait Kuwait City before resorting to litigation in the competent Kuwaiti courts.
8. The Radiologist shall carry professional medical malpractice insurance with a minimum coverage of 500,000 KWD, valid throughout the term of this Purchase Order, and shall provide a certificate of insurance upon request.
9. This Purchase Order is governed by the laws of the State of Kuwait. All references to the place of performance refer to the operational facility located in Kuwait Kuwait City, Kingdom of Kuwait.

By signing below, both parties acknowledge that they have read, understood, and agree to all terms set forth in this Purchase Order for the engagement of a Radiologist in Kuwait Kuwait City. This document is executed in two (2) original copies, one for each party.
