This Purchase Order authorizes the procurement of qualified School Counselor professionals to serve students across multiple Chicago Public Schools locations in the United States Chicago metropolitan area. Each School Counselor engaged under this Purchase Order must hold a valid Illinois School Counselor license issued by the Illinois State Board of Education and demonstrate a minimum of three (3) years of experience in urban K-12 counseling settings. The School Counselor personnel shall adhere to the American School Counselor Association (ASCA) ethical standards and the specific behavioral health protocols established by the Chicago Board of Education. All services rendered under this Purchase Order shall be delivered within the United States Chicago school district boundaries, with primary service locations at the schools designated in Appendix A of the master service agreement. The vendor shall ensure that each School Counselor maintains a student-to-counselor ratio not exceeding 250:1, in compliance with Illinois Public Act 100-0554 and United States Chicago district policy.
Terms and Conditions1. Compliance: All School Counselor services procured under this Purchase Order shall comply with all applicable federal, state (Illinois), and local (United States Chicago) laws, including but not limited to FERPA, IDEA, and the Illinois School Code (105 ILCS 5/).
2. Background Checks: Prior to commencing services, each School Counselor must pass a fingerprint-based background check through the Illinois State Police and the United States Chicago Metropolitan Area Agency on Crime (MAAC). No School Counselor shall access student records or enter school premises until clearance is confirmed.
3. Performance Standards: The vendor shall provide quarterly performance reports to the Chicago Board of Education detailing School Counselor caseloads, intervention outcomes, parent engagement metrics, and compliance with the United States Chicago Student Support Services framework.
4. Termination: Either party may terminate this Purchase Order with thirty (30) days written notice. The Chicago Board of Education reserves the right to terminate immediately for cause, including but not limited to failure of a School Counselor to maintain licensure, ethical violations, or failure to meet the performance benchmarks outlined in Section 3.
5. Insurance: The vendor shall maintain professional liability insurance of no less than $2,000,000 per occurrence and $5,000,000 aggregate, naming the Chicago Board of Education as an additional insured party for all School Counselor activities conducted in United States Chicago.
6. Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the State of Illinois and the municipal ordinances of the City of Chicago, United States Chicago.
7. Non-Discrimination: All School Counselor services shall be provided in strict compliance with Title VI, Title IX, Section 504 of the Rehabilitation Act, and the Americans with Disabilities Act, ensuring equitable access for all students in United States Chicago regardless of race, gender, disability, or socioeconomic status.
