The School Counselor shall report for duty no later than August 18, 2025, at the Westside Academic Campus located at 4800 W. Century Boulevard, Los Angeles, California 90045, United States. All onboarding, background check clearance (per California Education Code §45125), and credential verification must be completed prior to the first day of instruction. The service period covered by this Purchase Order extends from August 18, 2025, through June 12, 2026, encompassing one full academic year.
Payment shall be made via electronic funds transfer (EFT) to the vendor account on file. Invoices are due within thirty (30) days of receipt. The School Counselor compensation (Line Item 1) shall be disbursed in twelve (12) equal monthly installments. All other line items shall be invoiced upon completion of the respective service or delivery. Late payments shall accrue interest at a rate of 1.5% per month in accordance with California Government Code §11350.3. This Purchase Order is subject to the availability of appropriated funds from the FY 2025–2026 Student Services budget allocation.
- This Purchase Order constitutes a binding agreement between the Los Angeles Unified School District and the named vendor upon signature by both authorized parties. All services must be rendered in compliance with California state education law, the Individuals with Disabilities Education Act (IDEA), and all applicable federal and local regulations governing student counseling services in United States Los Angeles.
- The School Counselor shall maintain strict confidentiality of all student records in accordance with the Family Educational Rights and Privacy Act (FERPA) and California Education Code §49068. Any breach of confidentiality shall constitute grounds for immediate termination of this Purchase Order.
- The vendor warrants that the School Counselor assigned under this Purchase Order holds an active, unrestricted California School Counselor credential issued by the CTC and shall maintain said credential in good standing throughout the contract period.
- The vendor shall provide a replacement School Counselor of equivalent or superior qualifications within ten (10) business days should the originally assigned professional become unavailable due to resignation, medical leave, or disciplinary action.
- All services described herein shall be performed exclusively within the geographic boundaries of the City of Los Angeles, California, United States, unless prior written authorization is obtained from the Procurement Officer.
- This Purchase Order may be amended only by mutual written agreement of both parties. No oral modifications shall be binding. Any amendment must be documented on a formal Change Order referencing PO-LA-2025-04872.
- The vendor shall carry professional liability insurance with a minimum coverage of $2,000,000 per occurrence and shall provide a Certificate of Insurance to the District prior to the commencement of services.
- Disputes arising under this Purchase Order shall be resolved through mediation in Los Angeles County, California, in accordance with the California Uniform Arbitration Act. The prevailing party shall be entitled to recover reasonable attorney's fees.
- This Purchase Order is governed by the laws of the State of California and the federal laws of the United States of America. The exclusive venue for any legal proceedings shall be the Superior Court of California, County of Los Angeles.
