This Purchase Order authorizes the procurement of professional School Counselor services to be delivered within the United States San Francisco school district. The contracted School Counselor shall provide comprehensive student support services in alignment with the California Department of Education standards and the San Francisco Unified School District's Student Support Services framework. The School Counselor will be assigned to Lincoln Middle School, located at 1200 24th Avenue, San Francisco, California 94116, United States.
The scope of services for the School Counselor includes, but is not limited to: individual and group counseling sessions for students in grades 6 through 8; academic planning and career guidance; crisis intervention and mental health support; collaboration with teachers, parents, and administrators; development and implementation of a comprehensive school counseling program; and compliance with all applicable California Education Code provisions governing student counseling services in the United States San Francisco district.

Payment for this Purchase Order shall be made in accordance with the following schedule. All payments will be processed through the San Francisco Unified School District Accounts Payable department located in San Francisco, California, United States. The School Counselor services shall be invoiced on a monthly basis, with payment due within thirty (30) days of invoice receipt. The initial payment of $8,513.50 (representing one-tenth of the total) shall be due upon the School Counselor's first day of service on August 18, 2025. Subsequent monthly payments of $8,513.50 shall be due on the first business day of each month. The final payment shall be due no later than July 15, 2026. All payments shall be made via electronic funds transfer (EFT) to the vendor account on file. Late payments shall accrue interest at a rate of 1.5% per month as stipulated under California Public Contract Code Section 7264, applicable to public entities in the United States San Francisco jurisdiction.
VI. Terms and Conditions- This Purchase Order constitutes a binding agreement between the San Francisco Unified School District and Bay Area Educational Consulting Group, LLC for the provision of School Counselor services within the United States San Francisco school district.
- The School Counselor must hold a valid California Preliminary or Clear Pupil Personnel Services (PPS) Credential issued by the California Commission on Teacher Credentialing (CTC) and must maintain this credential throughout the duration of this Purchase Order.
- The School Counselor shall comply with all federal, state, and local laws and regulations applicable to student counseling services in the State of California, United States, including but not limited to FERPA, IDEA, and California Education Code Sections 49400 through 49499.
- The vendor shall maintain professional liability insurance with a minimum coverage of $1,000,000 per occurrence and $2,000,000 aggregate, naming the San Francisco Unified School District as an additional insured party.
- This Purchase Order may be terminated by either party with thirty (30) days written notice. In the event of termination, the School Counselor shall complete all in-progress counseling sessions and provide a comprehensive transition report to the district.
- All student records, counseling notes, and confidential information handled by the School Counselor shall remain the property of the San Francisco Unified School District and shall be stored in accordance with California Confidentiality of Medical Information Act (CMIA) and FERPA requirements.
- The School Counselor shall report directly to the Principal of Lincoln Middle School and the Director of Student Support Services, San Francisco Unified School District, San Francisco, United States.
- This Purchase Order is governed by the laws of the State of California, United States. Any disputes arising from this Purchase Order shall be resolved through binding arbitration in San Francisco, California, in accordance with the California Arbitration Act.
- The vendor acknowledges that this Purchase Order is subject to the availability of appropriated funds by the San Francisco Unified School District Board of Education. If funds are not available, this Purchase Order may be cancelled without penalty.
- All services under this Purchase Order shall be performed in San Francisco, California, United States, and the School Counselor shall be physically present at the assigned school site during all contracted hours.
