Description of Services – Social Worker EngagementThis Purchase Order is issued by Toronto Community Wellness Services Inc. to formally authorize the procurement of professional Social Worker services for the duration of the fiscal year 2025–2026. 1. Scope of Engagement: The Social Worker(s) engaged under this Purchase Order shall operate in full compliance with the Ontario Social Work Act, 1998, and all regulations established by the Ontario College of Social Workers and Social Workers (OCSW). All services shall be delivered within the geographic boundaries of the City of Toronto, Canada, unless otherwise authorized in writing by the purchasing authority.
2. Qualifications: The Social Worker must hold a valid registration with the Ontario College of Social Workers and Social Workers (OCSW) and possess a minimum of five (5) years of post-graduate experience in community-based social work practice in Canada. The Social Worker must demonstrate proficiency in culturally sensitive service delivery, with particular attention to the diverse populations residing in Toronto, Ontario.
3. Payment Schedule: Invoices shall be submitted monthly on the last business day of each calendar month. Payment shall be processed within thirty (30) days of receipt of a valid invoice, in accordance with the Payment Terms specified in this Purchase Order. All payments shall be made in Canadian Dollars (CAD) via electronic funds transfer (EFT) to the account designated by the vendor.
4. Performance Standards: The Social Worker shall adhere to the professional code of ethics established by the OCSW and shall maintain all required professional liability insurance with a minimum coverage of $5,000,000 per occurrence. Quarterly performance reviews shall be conducted by Toronto Community Wellness Services Inc. to ensure service quality meets the standards expected of social work professionals operating in Canada Toronto.
5. Confidentiality and Data Protection: All client information handled by the Social Worker shall be protected in accordance with the Personal Information Protection and Electronic Documents Act (PIPEDA) and the Ontario Freedom of Information and Protection of Privacy Act (FIPPA). No client records shall be transferred outside of Canada without explicit written consent.
6. Termination: Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination, the Social Worker shall complete all active case files and provide a comprehensive transition report to the purchasing organization within fourteen (14) days of the termination effective date.
7. Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the Province of Ontario and the federal laws of Canada applicable therein. Any disputes arising from this Purchase Order shall be resolved through mediation in Toronto, Ontario, before proceeding to litigation in the Superior Court of Justice for the Province of Ontario.
8. Compliance: The vendor and the Social Worker shall comply with all applicable federal, provincial, and municipal regulations in Canada, including but not limited to the Canada Labour Code, the Ontario Employment Standards Act, 2000, and the City of Toronto's municipal bylaws governing community service providers.
