- This Purchase Order constitutes a binding agreement between the Accra Metropolitan Community Development Authority and Horizon Social Services & Counselling Ltd. for the provision of professional Social Worker services within the Ghana Accra metropolitan area.
- The Social Worker assigned to this contract must hold a valid registration with the Ghana Social Work Council (GSWC) and possess a minimum of five (5) years of post-qualification experience in community-based social work practice in Ghana.
- Payment shall be made in monthly installments of GHS 18,170.00 (inclusive of VAT) via bank transfer to the Service Provider's designated account, within fourteen (14) calendar days of receipt of an approved monthly invoice and corresponding progress report.
- The Service Provider shall ensure that the Social Worker adheres to the Code of Ethics of the Ghana Social Work Council and the National Social Protection Policy of the Republic of Ghana at all times during the engagement.
- All client data and case records generated by the Social Worker shall be treated as strictly confidential in accordance with the Data Protection Act, 2012 (Act 843) of Ghana. No records shall be disclosed to third parties without written consent from the AMCDA Director of Social Services.
- This Purchase Order may be terminated by either party with thirty (30) days' written notice. In the event of termination, the Service Provider shall be compensated for all services rendered up to the effective date of termination, less any outstanding obligations.
- Any disputes arising from this Purchase Order shall be resolved through mediation in Accra, Ghana, in accordance with the Alternative Dispute Resolution Act, 2010 (Act 798). Failing mediation, disputes shall be referred to the courts of competent jurisdiction in Ghana Accra.
- The Service Provider shall maintain professional indemnity insurance of not less than GHS 500,000.00 for the duration of this Purchase Order, covering all activities performed by the Social Worker in the course of duty.
- All services under this Purchase Order shall be performed within the administrative boundaries of the Accra Metropolitan Assembly, Ghana Accra, unless prior written approval is obtained from the Buyer for out-of-area assignments.
- This Purchase Order is governed by the laws of the Republic of Ghana, including the Public Procurement Act, 2003 (Act 663) and its amendments.
