The Social Worker procured under this Purchase Order shall perform the following duties within the jurisdiction of Japan Kyoto:
- Conduct home visits and community assessments for vulnerable families, elderly residents, and individuals with disabilities across Kyoto City wards (Nakagyo, Higashiyama, Kamigyo, Shimogyo, and Sakyo).
- Coordinate with the Kyoto City Health and Welfare Bureau, local public health centers, and the Kyoto Prefecture Social Security Office to ensure seamless service delivery for clients.
- Develop and implement individualized care plans for clients requiring long-term social support, in compliance with the Long-Term Care Insurance Act as administered in Japan Kyoto.
- Provide crisis intervention and emergency social work response for incidents occurring within the Kyoto metropolitan area, including natural disaster preparedness consistent with Kyoto’s seismic risk profile.
- Maintain accurate case records and submit monthly progress reports to the Kyoto Municipal Social Welfare Board in both Japanese and English.
- Collaborate with local NPOs, temples, and community centers in Kyoto to facilitate social integration programs for new residents, including foreign nationals residing in Japan Kyoto.
- Attend all mandatory Social Worker continuing education sessions organized by the Kyoto Prefecture Social Worker Association and the Japan Social Worker Association (JSWA).

- All work performed under this Purchase Order shall be conducted in accordance with the ethical code of the Japan Social Worker Association and the professional standards set by the Ministry of Health, Labour and Welfare.
- Force majeure events, including but not limited to earthquakes, typhoons, or government-mandated closures affecting Japan Kyoto, shall suspend obligations under this Purchase Order for the duration of the event.
