- This Purchase Order is issued by the Manila Community Development Foundation, Inc. for the procurement of professional Social Worker services to be rendered within the Philippines Manila metropolitan area. All Social Worker personnel deployed under this Purchase Order must hold a valid Professional Regulation Commission (PRC) license as a Registered Social Worker in the Philippines.
- The vendor, Philippine Social Work Services Consortium, Inc., shall ensure that every Social Worker assigned to this contract meets the minimum qualifications stipulated by the Department of Social Welfare and Development (DSWD) and the Philippine Social Work Act (R.A. 9138), including a bachelor's degree in Social Work and a minimum of three (3) years of relevant field experience in the Philippines Manila region.
- Payment shall be made in Philippine Pesos (PHP) via bank transfer to the vendor's designated account within thirty (30) calendar days from the receipt of a valid invoice. Late payments shall accrue interest at the rate of 1.5% per month as prescribed by the Philippine Civil Code.
- The Social Worker services shall commence on July 1, 2025, and shall continue for a period of twelve (12) months unless terminated earlier by mutual written agreement or in accordance with the termination clauses herein. The primary service area is the Philippines Manila region, encompassing the 16 cities and municipalities of Metro Manila.
- The vendor shall provide monthly progress reports detailing the activities of each Social Worker, including number of beneficiaries served, case outcomes, and any incidents requiring escalation. Reports shall be submitted to the foundation's program office in Makati City, Philippines Manila, by the fifth (5th) business day of the following month.
- Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, the vendor shall be compensated for all Social Worker services rendered up to the effective date of termination, less any applicable deductions.
- All Social Worker personnel shall comply with the Data Privacy Act of 2012 (Republic Act No. 10173) of the Philippines and shall maintain strict confidentiality regarding all beneficiary information collected during the course of service delivery in the Philippines Manila area.
- This Purchase Order shall be governed by and construed in accordance with the laws of the Republic of the Philippines. Any disputes arising herefrom shall be resolved through mediation before the Philippine Center for International Investment and Arbitration (PCIA) in Manila, Philippines Manila.
