3. Description of Services and Line Items

4. Terms and Conditions

Scope of Services: The Speech Therapist shall provide all clinical services in accordance with the professional standards of the British Columbia College of Speech-Language Pathologists and Audiologists. All therapy plans must be individualized and evidence-based.
Service Location: All services under this Purchase Order shall be rendered at the Chen Speech & Language Therapy Clinic located in Canada Vancouver, or at such other locations within the Canada Vancouver metropolitan area as mutually agreed upon in writing.
Qualifications: The Speech Therapist must maintain active registration with the relevant Canadian regulatory body and hold a minimum of five (5) years of post-graduate clinical experience in speech-language pathology.
Confidentiality: The Speech Therapist shall comply with all provisions of the Personal Information Protection Act (PIPA) of British Columbia and the federal Privacy Act of Canada regarding the handling of patient records and clinical data.
Payment Schedule: Invoices shall be submitted monthly on the last business day of each calendar month. Payment is due within thirty (30) days of receipt of a valid invoice. Late payments shall accrue interest at the rate prescribed under the British Columbia Interest Act.
Termination: Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination, the Speech Therapist shall complete all in-progress therapy sessions and deliver final reports within fourteen (14) days.
Insurance: The Speech Therapist shall maintain professional liability insurance with a minimum coverage of $2,000,000 per occurrence, issued by a Canadian insurance carrier, for the duration of this Purchase Order.
Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the Province of British Columbia and the federal laws of Canada applicable therein.
Dispute Resolution: Any disputes arising from this Purchase Order shall first be subject to mediation in Canada Vancouver. If mediation is unsuccessful, disputes shall be resolved through arbitration under the Arbitration Act of British Columbia.
Compliance: The Speech Therapist warrants that all services will comply with the Canadian Health Information Protection regulations and any applicable municipal bylaws in the City of Vancouver.

5. Acceptance and Authorization

By signing below, both parties acknowledge and agree to the terms, conditions, and service specifications outlined in this Purchase Order for Speech Therapist services in Canada Vancouver. This document constitutes a binding agreement effective from the date of the last signature.
