1. Scope of Service: The Speech Therapist shall render all services listed in this Purchase Order at the Clínica Integrada del Pacífico SpA premises in Santiago, Chile, or at such other location within the Santiago Metropolitan Region as mutually agreed in writing. All clinical interventions must comply with the Chilean Health Ministry (Ministerio de Salud) regulations and the professional code of ethics of the Colegio de Logopedas de Chile.
2. Service Location: All in-person therapy sessions, assessments, and workshops shall be conducted at the facility address listed above in Chile Santiago, specifically at Av. Apoquindo 4520, Las Condes. Remote teletherapy sessions, if authorized, shall be limited to a maximum of 20% of the total scheduled sessions and must be documented in the patient's electronic health record.
3. Payment Schedule: Invoices shall be submitted monthly by the 5th business day of the following month. Payment shall be made via electronic bank transfer (transferencia electrónica) within thirty (30) calendar days of invoice receipt.
