This Purchase Order is issued by Meridian Corporate Wellness Solutions Pvt. Ltd. to formally engage the services of a qualified Speech Therapist for the duration specified below. All services under this Purchase Order shall be rendered at the Clarity Speech & Language Therapy Centre premises in India Bangalore, or at the Meridian Corporate Wellness Solutions office in Whitefield, India Bangalore, as mutually agreed upon by both parties. The Speech Therapist is expected to maintain a minimum of three (3) working days per week at the designated India Bangalore location. Any change in service delivery location within the India Bangalore metropolitan area shall require prior written approval from the purchasing organization.
5. Terms and Conditions- This Purchase Order constitutes a binding agreement between the purchasing organization and the service provider for the engagement of a Speech Therapist in India Bangalore.
- The Speech Therapist shall hold valid registration with the Association of Speech-Language Pathologists and Audiology (ASLPA) and shall maintain professional liability insurance throughout the engagement period.
- Payment shall be made within thirty (30) days of receipt of a valid tax invoice from the service provider, via NEFT/RTGS to the bank account specified in this Purchase Order.
- The service provider shall comply with all applicable data protection regulations under the Digital Personal Data Protection Act, 2023, particularly regarding the handling of patient records in India Bangalore.
- Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of early termination, payment shall be made only for services rendered up to the date of termination.
- The Speech Therapist shall submit monthly activity reports detailing sessions conducted, patient progress, and recommendations to the HR Wellness team in India Bangalore.
- All disputes arising from this Purchase Order shall be subject to the jurisdiction of the competent courts in India Bangalore, Karnataka, India.
- The service provider shall not subcontract the Speech Therapist role to any third party without prior written consent from the purchasing organization.
- This Purchase Order is valid for a period of twelve (12) months from the date of issue, unless renewed in writing by both parties.
