This Purchase Order authorizes the engagement of qualified Speech Therapist professionals and associated services to be rendered at the Al-Nahda Medical Center facility located in Kuwait Kuwait City. The Speech Therapist(s) shall provide comprehensive speech-language pathology services in accordance with the standards set by the Kuwait Ministry of Health and the American Speech-Language-Hearing Association (ASHA) guidelines. All clinical documentation, patient assessments, and therapy plans must be maintained in both Arabic and English. The Speech Therapist is required to attend weekly multidisciplinary team meetings and submit monthly progress reports to the supervising physician. All services must be delivered within the premises in Kuwait Kuwait City unless prior written authorization is granted by the buyer.
- This Purchase Order is governed by the laws of the State of Kuwait. Any disputes arising from this Purchase Order shall be resolved through the competent courts in Kuwait Kuwait City.
- The Speech Therapist must hold a valid professional license issued by the Kuwait Ministry of Health and maintain professional indemnity insurance with a minimum coverage of KWD 50,000 for the duration of this contract.
- Payment shall be processed via bank transfer to the supplier's designated account within Kuwait Kuwait City within thirty (30) calendar days of receipt of a valid tax invoice.
- The supplier shall provide a replacement Speech Therapist of equivalent qualifications within five (5) business days in the event of the primary Speech Therapist's unavailability due to illness or other circumstances.
- All patient data and clinical records generated during the provision of Speech Therapist services shall remain the exclusive property of Al-Nahda Healthcare Services LLC and must comply with Kuwait's data protection regulations.
- This Purchase Order may be terminated by either party with thirty (30) days' written notice. In the event of termination, the supplier shall be compensated for services rendered up to the termination date.
- Any modifications to this Purchase Order must be made in writing and signed by both parties. Verbal agreements shall not be binding.
- The supplier warrants that all Speech Therapist personnel assigned under this Purchase Order are free from any conflict of interest and are not under any disciplinary action by the Kuwait Medical Specialties Board.
By signing below, both parties acknowledge and agree to the terms, conditions, and scope of work outlined in this Purchase Order for the provision of Speech Therapist services in Kuwait Kuwait City.
