1. Description of Purchase Order and Scope of Services

This Purchase Order is issued by Al-Noor Children's Rehabilitation & Development Centre, Pakistan Karachi, to formally engage the services of a qualified Speech Therapist for the provision of comprehensive speech-language pathology, articulation therapy, and communication rehabilitation services to pediatric and adult patients. 3. Terms and Conditions of the Purchase Order
  1. Validity: This Purchase Order shall remain valid for a period of twelve (12) months from the date of acceptance, commencing on 01 July 2025 and concluding on 30 June 2026, unless renewed in writing by both parties.
  2. Qualifications: The Speech Therapist must hold a Master's degree in Speech-Language Pathology from a recognized institution in Pakistan Karachi or an internationally accredited university, and must maintain active registration with the Sindh Medical and Dental Council. Proof of credentials shall be submitted prior to commencement of services.
  3. Payment Terms: Payment shall be made on a monthly basis within fifteen (15) working days of receipt of the Speech Therapist's invoice and verified service log. Payments shall be processed via bank transfer to the account designated by the Speech Therapist. A withholding tax of 15% shall be deducted as per the Federal Board of Revenue (FBR) regulations applicable in Pakistan Karachi.
  4. Service Location: All clinical services under this Purchase Order shall be rendered at the Al-Noor Centre premises in Pakistan Karachi, Clifton Block 5, unless a home visit has been specifically authorized in writing by the centre's Medical Director.
  5. Confidentiality: The Speech Therapist shall maintain strict confidentiality of all patient records, clinical notes, and personal data in compliance with the Pakistan Personal Data Protection Bill and institutional policies. No patient information shall be disclosed to third parties without written consent.
  6. Performance Standards: The Speech Therapist is expected to maintain a minimum patient attendance rate of 85% per scheduled week. Failure to meet this threshold for two consecutive months shall constitute grounds for review of this Purchase Order.
  7. Termination: Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, the Speech Therapist shall be compensated for all services rendered up to the effective date of termination.
  8. Dispute Resolution: Any disputes arising from this Purchase Order shall be resolved through amicable negotiation. If unresolved, the matter shall be referred to the competent civil court in Pakistan Karachi, Sindh, in accordance with the Code of Civil Procedure, 1908.
  9. Force Majeure: Neither party shall be liable for failure to perform obligations under this Purchase Order due to events beyond reasonable control, including but not limited to natural disasters, government-imposed lockdowns in Pakistan Karachi, or public health emergencies.
  10. Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the Islamic Republic of Pakistan, specifically the Sindh provincial regulations pertaining to allied health services.
4. Acceptance and Authorization

By signing below, both parties acknowledge that they have read, understood, and agree to all terms and conditions outlined in this Purchase Order for the engagement of the Speech Therapist in Pakistan Karachi. This document represents the complete and final agreement between the parties regarding the scope, compensation, and obligations described herein.
