The Speech Therapist engaged under this Purchase Order shall perform the following duties and responsibilities within the premises of Manila Children's Development Center, Inc., located in Philippines Manila:
- Conduct comprehensive speech, language, and communication assessments using validated instruments recognized by the Philippine Department of Health and the American Speech-Language-Hearing Association (ASHA).
- Develop and implement individualized treatment plans (ITPs) for each patient, with progress reviews conducted every four (4) weeks.
- Deliver direct therapy sessions in a child-friendly, culturally sensitive environment appropriate for the diverse communities served in Philippines Manila.
- Maintain accurate and confidential patient records in accordance with the Philippine Data Privacy Act of 2012 (Republic Act No. 10173).
- Participate in multidisciplinary team meetings and contribute to the overall developmental care plan of each child.
- Provide monthly written progress reports to the Center's Medical Director and to the parents or legal guardians of the patients.
- Ensure all therapy materials and equipment are sanitized and maintained in accordance with the infection control protocols of the facility.
4.1 Payment Schedule
Payment for the services described in this Purchase Order shall be made in monthly installments. The vendor shall submit a detailed invoice at the end of each calendar month. Payment shall be processed within thirty (30) days from the date of receipt of a valid invoice. All payments shall be made in Philippine Pesos (PHP) via bank transfer to the account designated by the vendor.
4.2 Qualification Requirements
The Speech Therapist assigned to this engagement must hold a valid Professional Regulation Commission (PRC) license as a Speech-Language Pathologist in the Republic of the Philippines, a Bachelor's or Master's degree in Speech-Language Pathology or a related field from a CHED-recognized institution, and a minimum of three (3) years of clinical experience in pediatric speech therapy. Proof of licensure and credentials shall be submitted prior to the commencement of services.
4.3 Commencement and Duration
Services shall commence on July 1, 2025, and shall continue for a period of twelve (12) months, ending on June 30, 2026, unless terminated earlier in accordance with Section 4.5. The Speech Therapist shall report to the facility in Philippines Manila on a full-time basis, Monday through Friday, from 8:00 AM to 5:00 PM, with a one-hour lunch break.
4.4 Termination
Either party may terminate this Purchase Order by providing thirty (30) days' written notice to the other party. In the event of material breach, the non-breaching party may terminate immediately upon written notice. Upon termination, the vendor shall be compensated for all services rendered up to the effective date of termination.
4.5 Compliance with Philippine Law
All services rendered under this Purchase Order shall comply with the laws of the Republic of the Philippines, including but not limited to the Labor Code of the Philippines, the Data Privacy Act of 2012, the Magna Carta of the Disabled (Republic Act No. 7277), and all applicable regulations of the Department of Health and the Professional Regulation Commission. The vendor shall bear all applicable taxes, including Value-Added Tax (VAT) at the prevailing rate of 12%, which shall be itemized separately on each invoice.
4.6 Confidentiality
The vendor and the assigned Speech Therapist shall maintain strict confidentiality of all patient information, clinical records, and proprietary treatment methodologies. Any breach of confidentiality shall constitute grounds for immediate termination of this Purchase Order and may result in legal action under Philippine law.
