Description of Services – Speech Therapist Engagement

This Purchase Order is issued by Dar es Salaam Health & Wellness Services Ltd. to formally engage the services of a qualified Speech Therapist to provide comprehensive speech and language therapy services to patients and clients within the Dar es Salaam region of Tanzania. The Speech Therapist shall be responsible for the assessment, diagnosis, treatment, and rehabilitation of individuals with speech, language, voice, fluency, and swallowing disorders. The services shall be delivered in accordance with the professional standards set by the Tanzania Medical and Dental Council and the International Speech-Language Association. The Speech Therapist is expected to work closely with the multidisciplinary clinical team at our facility in Dar es Salaam, Tanzania, to ensure holistic patient care and optimal therapeutic outcomes.

Terms and Conditions

This Purchase Order is valid for acceptance within fourteen (14) calendar days from the date of issue. Failure to accept within this period shall render this Purchase Order null and void.
The Speech Therapist engaged under this Purchase Order must hold a valid professional license issued by the Tanzania Medical and Dental Council and a minimum of a Bachelor's degree in Speech and Language Therapy or a related field from a recognized institution in Tanzania or internationally.
All services shall be rendered at the designated facility in Dar es Salaam, Tanzania, unless otherwise agreed in writing by both parties. The Speech Therapist shall comply with all local health regulations, infection control protocols, and data protection laws applicable in Tanzania.
Payment shall be made via bank transfer to the account specified by the supplier within thirty (30) days of receipt of a valid invoice. Late payments shall attract interest at the rate of 2% per month as stipulated under Tanzanian commercial law.
The Speech Therapist shall maintain full professional confidentiality regarding all patient information in accordance with the Tanzania Data Protection Act, 2022, and shall not disclose any client records without written consent.
Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, the supplier shall be compensated for all services rendered up to the date of termination.
All disputes arising from this Purchase Order shall be resolved through amicable negotiation. Should negotiation fail, the matter shall be referred to arbitration in Dar es Salaam, Tanzania, in accordance with the Arbitration Act, Cap. 20 of the Laws of Tanzania.
The Speech Therapist shall submit a comprehensive end-of-contract report summarizing all therapeutic outcomes, patient progress, and recommendations for continued care within fourteen (14) days of the contract expiry date.
This Purchase Order is governed by and shall be construed in accordance with the laws of the United Republic of Tanzania.
