This Purchase Order is issued by the Tashkent Regional Children's Rehabilitation Center to formally request and authorize the engagement of a qualified Speech Therapist to provide comprehensive speech-language pathology services to children and adolescents within the Tashkent metropolitan area of Uzbekistan. The Speech Therapist shall be a licensed professional holding a minimum of a Master's degree in Speech-Language Pathology, registered with the Ministry of Health of the Republic of Uzbekistan, and possessing no fewer than five (5) years of documented clinical experience in pediatric speech therapy.
The scope of services under this Purchase Order includes, but is not limited to: initial speech and language assessments for children aged 2 to 17; development and implementation of individualized therapy plans; direct one-on-one and group speech therapy sessions; articulation and phonological intervention; fluency disorder treatment; voice therapy; language comprehension and expression programs; and regular progress reporting to the center's medical board. All services shall be delivered at the designated facility in Uzbekistan Tashkent or at approved satellite clinics within the Tashkent city limits.

5.1 This Purchase Order constitutes a binding agreement between the Buyer and the Supplier for the provision of Speech Therapist services in Uzbekistan Tashkent. All terms herein shall be governed by the Civil Code of the Republic of Uzbekistan and applicable commercial regulations.
5.2 The Speech Therapist assigned under this Purchase Order must maintain valid professional licensure throughout the contract period. Any lapse in certification shall result in immediate suspension of services pending reinstatement.
5.3 The Supplier shall ensure that all Speech Therapist personnel comply with the data protection and patient confidentiality laws of Uzbekistan. All patient records generated in Tashkent shall be stored in accordance with the Republic of Uzbekistan's Health Data Protection Regulations.
5.4 Payment shall be processed via bank transfer to the Supplier's designated account in Tashkent, Uzbekistan. Invoices must reference this Purchase Order number (UZ-TSK-2025-04871) for proper accounting reconciliation.
5.5 Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, the Supplier shall provide a comprehensive handover report for all active Speech Therapist caseloads to ensure continuity of care for patients in Uzbekistan Tashkent.
5.6 The Supplier warrants that the Speech Therapist services shall meet the clinical standards established by the Uzbekistan National Health Standards for Pediatric Rehabilitation and shall be subject to quarterly performance audits by the Buyer's quality assurance team.
By signing below, both parties acknowledge and accept all terms, conditions, and service specifications outlined in this Purchase Order for the engagement of a Speech Therapist in Uzbekistan Tashkent. This document is valid for a period of twelve (12) months from the date of the last signature and may be renewed upon mutual written consent.
