IMPORTANT NOTE: All surgical procedures performed by the Surgeon in Colombia Bogotá must adhere to the Colombian General Health System (SGSSS) regulations. TERMS AND CONDITIONS
  1. Scope of Work: The Surgeon shall provide all surgical services as outlined in this Purchase Order at the Clínica San Rafael facility located in Colombia Bogotá. The Surgeon must maintain a minimum of 120 operative hours per contract period and be available for emergency call-outs with a response time not exceeding 45 minutes.
  2. Regulatory Compliance: The Surgeon must possess and maintain a valid Colombian medical habilitation (Régimen Especial) issued by the Ministry of Health and Social Protection. All equipment and materials referenced in this Purchase Order must carry valid INVIMA registration numbers for use in Colombia Bogotá.
  3. Delivery and Installation: All physical goods (surgical kits, consumables) shall be delivered to the Colombia Bogotá facility no later than July 1, 2025. Technical support for the Surgeon and hospital staff in Colombia Bogotá shall be available via phone and video conference within 4 business hours of any reported issue.
  6. Confidentiality: Both parties agree to maintain strict confidentiality regarding patient data, surgical techniques, and proprietary information. All patient records generated in Colombia Bogotá shall be handled in accordance with Ley 1581 de 2012 (Colombian Data Protection Law) and the hospital's internal data governance policies.
  7. Termination: Either party may terminate this Purchase Order with 30 days' written notice.
