Total Purchase Order Value: USD 101,350.00 (One Hundred and One Thousand Three Hundred and Fifty US Dollars)

Equivalent in Iraqi Dinar at the Central Bank of Iraq reference rate on the date of issue: IQD 131,755,000 (One Hundred and Thirty-One Million Seven Hundred and Fifty-Five Thousand Iraqi Dinars).

The Surgeon engaged under this Purchase Order shall:

  • Report to the Al-Rafidain General Medical Center surgical ward in Baghdad, Iraq, no later than 07:00 AM on all scheduled working days (Sunday through Thursday).
  • Perform all surgical procedures in strict compliance with the Iraqi Ministry of Health clinical guidelines and the facility's internal surgical protocols.
  • Maintain complete and accurate patient records, operative notes, and post-operative reports in the hospital's electronic medical system.
  • Be available for emergency surgical interventions in Baghdad, Iraq, during the on-call rotation period specified in Item 4 above.
  • Provide written surgical reports within 48 hours of each procedure performed.
  • Participate in weekly multidisciplinary case conferences held at the Baghdad facility.
  • Maintain valid malpractice and professional liability insurance coverage throughout the contract period.
  • Adhere to all infection control, sterilization, and patient safety standards mandated by the Iraqi Ministry of Health for surgical facilities in Baghdad.
  1. Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the Republic of Iraq, including the Iraqi Civil Code and applicable healthcare regulations issued by the Iraqi Ministry of Health.
  2. Payment Terms: Payment shall be made in monthly installments within thirty (30) calendar days of receipt of an itemized invoice from the Surgeon. Payment shall be processed via bank transfer to the account designated by the Surgeon. All payments are subject to applicable Iraqi tax withholdings as determined by the Iraqi General Commission for Taxation.
  3. Commencement and Duration: Services shall commence on 1 July 2025 and shall continue for a period of three (3) months, subject to renewal by mutual written agreement. The Surgeon must be physically present in Baghdad, Iraq, for the entire contract duration.
  4. Termination: Either party may terminate this Purchase Order with thirty (30) days' written notice. The issuing organization may terminate immediately for cause, including but not limited to gross negligence, violation of Iraqi medical ethics codes, or failure to maintain required licensure.
  5. Confidentiality: The Surgeon shall maintain strict confidentiality of all patient information in accordance with Iraqi data protection regulations and the facility's internal privacy policy. Breach of patient confidentiality in Baghdad, Iraq, shall constitute grounds for immediate termination and potential legal action.
  6. Dispute Resolution: Any dispute arising from this Purchase Order shall first be subject to good-faith negotiation. If unresolved within thirty (30) days, the matter shall be referred to the competent courts in Baghdad, Iraq.
  7. Force Majeure: Neither party shall be liable for failure to perform obligations due to events beyond reasonable control, including but not limited to civil unrest, natural disasters, or government-mandated closures affecting medical facilities in Baghdad, Iraq.

This Purchase Order constitutes a formal and binding procurement document. By signing below, both parties acknowledge and agree to all terms, conditions, and service specifications outlined in this document regarding the engagement of the Surgeon at the facility in Iraq Baghdad.
