- This Purchase Order is issued by the Yangon Medical Procurement Authority for the engagement of a qualified Surgeon and the procurement of associated surgical supplies to be utilized exclusively within the administrative boundaries of Myanmar Yangon.
- The Surgeon named in this Purchase Order shall hold a valid medical license issued by the Myanmar Medical Council and shall maintain professional liability insurance with a minimum coverage of USD 500,000 for the duration of the contract.
- All surgical procedures referenced in this Purchase Order shall be conducted at Yangon Central Hospital or such other facility in Myanmar Yangon as mutually agreed upon in writing by both parties.
- Payment terms: 40% upon execution of this Purchase Order, 40% upon completion of all scheduled Surgeon procedures, and 20% upon final acceptance and submission of post-operative reports.
- The Surgeon shall comply with all applicable regulations of the Ministry of Health, Myanmar Yangon, including but not limited to infection control protocols, patient consent documentation, and medical waste disposal standards.
- Any modification to the scope of Surgeon services or the quantity of equipment listed in this Purchase Order must be submitted in writing and approved by the Director General before implementation.
- Disputes arising from this Purchase Order shall be resolved through arbitration in Myanmar Yangon in accordance with the Myanmar Arbitration Law.
- This Purchase Order shall be governed by the laws of the Republic of the Union of Myanmar, with the primary jurisdiction being Myanmar Yangon.
- The Surgeon shall submit a comprehensive final report detailing all procedures performed, outcomes, and recommendations within 30 days of the final scheduled operation.
- All equipment procured under this Purchase Order shall be delivered to the designated facility in Myanmar Yangon no later than 14 days prior to the first scheduled surgical date.
