This Purchase Order is issued by Capital Medical & Surgical Centre, Pakistan Islamabad, for the engagement of a qualified and licensed Surgeon to provide specialized surgical services at our facility. The Surgeon shall be responsible for performing general surgical procedures, laparoscopic operations, emergency surgical interventions, and post-operative patient management in accordance with the standards set by the Pakistan Medical & Dental Council (PMDC) and the regulatory authorities of Pakistan Islamabad.

- This Purchase Order constitutes a binding agreement between Capital Medical & Surgical Centre, Pakistan Islamabad, and the named Surgeon upon acceptance and signature by both parties.
- The Surgeon shall maintain a valid PMDC license throughout the duration of this engagement and shall provide proof of continued registration to the procurement office of Pakistan Islamabad upon request.
- Payment shall be made on a monthly basis within fifteen (15) calendar days of the end of each service month, via bank transfer to the account designated by the Surgeon.
- The Surgeon is required to be physically present at the Capital Medical & Surgical Centre premises in Pakistan Islamabad during all scheduled working hours and on-call shifts as specified in this Purchase Order.
- Any cancellation or early termination of this Purchase Order by either party requires a written notice of thirty (30) days. In the event of termination by the buyer without cause, a pro-rata payment for services rendered shall be made.
- The Surgeon shall adhere to all clinical protocols, infection control standards, and patient safety guidelines established by the facility and the regulatory bodies governing medical practice in Pakistan Islamabad.
- All surgical records, patient data, and clinical documentation generated under this Purchase Order shall remain the property of Capital Medical & Surgical Centre and shall be maintained in compliance with the Pakistan Data Protection Act.
- The Surgeon shall carry professional indemnity insurance with a minimum coverage of PKR 50,000,000, valid for the entire term of this Purchase Order.
- Disputes arising from this Purchase Order shall be resolved through arbitration in Pakistan Islamabad in accordance with the Arbitration Act, 1940 of Pakistan.
- This Purchase Order is governed by the laws of the Islamic Republic of Pakistan, and the courts of Pakistan Islamabad shall have exclusive jurisdiction.
The Surgeon shall commence duties at the Capital Medical & Surgical Centre, Pakistan Islamabad, on or before 1 July 2025. The initial setup of surgical instruments and consumables (Item No. 05) shall be delivered and installed no later than 28 June 2025. The full term of this Purchase Order shall run from 1 July 2025 to 30 June 2026, subject to renewal by mutual written agreement.
By signing below, both parties acknowledge that they have read, understood, and agree to all terms, conditions, and specifications outlined in this Purchase Order for the engagement of a Surgeon at the Capital Medical & Surgical Centre, Pakistan Islamabad.
