- This Purchase Order is issued by Meridian Animal Health & Wellness Centre, Melbourne, Victoria, Australia, and governs the engagement of Veterinarian Dr. Sarah Mitchell and her surgical team for the specified animal welfare services.
- All veterinary services shall be performed in strict compliance with the Victorian Animal Welfare Act 2013, the Companion Animals Act 1998 (Vic), and the standards set by the Veterinary Board of Australia (VBA).
- The Veterinarian and all assisting veterinary nurses must hold current registration with the Veterinary Board of Australia and valid professional indemnity insurance with a minimum coverage of AUD $10,000,000.
- Delivery of all pharmaceuticals, surgical supplies, and diagnostic reagents shall be made to the Melbourne facility at 42 Collins Street. The supplier is responsible for cold-chain integrity of all temperature-sensitive medications.

- The Veterinarian shall provide a comprehensive written summary report within five (5) business days of completion, detailing all procedures performed, medications administered, and recommended follow-up care for each animal.
- All records, medical files, and diagnostic imaging generated under this Purchase Order shall be retained for a minimum of seven (7) years in accordance with Australian veterinary record-keeping regulations and made available to the purchaser upon request.
- This Purchase Order is governed by the laws of the State of Victoria and the Commonwealth of Australia. Any disputes shall be resolved through the Victorian Small Claims Tribunal or, where the amount exceeds the jurisdictional limit, the Supreme Court of Victoria, Melbourne.
- The supplier warrants that all services will be performed with reasonable skill and care, and that all medications and supplies will be Australian Therapeutic Goods Administration (TGA) approved or otherwise legally permissible for veterinary use in Australia Melbourne.
