Terms and Conditions
- This Purchase Order is issued by Manila Veterinary Care Center, Inc., a licensed Veterinarian practice registered with the Philippine Veterinary Medical Association (PVMA) and the Professional Regulation Commission (PRC) in the Philippines Manila region. All veterinary supplies and equipment procured under this Purchase Order must comply with the standards set by the Food and Drug Administration (FDA) of the Philippines and the Bureau of Animal Industry (BAI).
- The supplier shall deliver all items listed in this Purchase Order to the designated delivery location in Metro Manila, Philippines, no later than the Required Delivery Date. Late delivery beyond five (5) business days shall entitle the Veterinarian practice to a penalty of 1% of the total Purchase Order value per day of delay, up to a maximum of 10%.
- All pharmaceutical and biological products (vaccines, antibiotics, IV fluids) must be accompanied by valid FDA Philippines registration certificates, batch numbers, and expiration dates. The Veterinarian in charge, Dr. Maria Santos, DVM, will inspect all deliveries upon arrival at the Manila facility and has the right to reject non-conforming items.
- Payment shall be made via bank transfer to the supplier's designated account within thirty (30) calendar days from the date of receipt of a valid Official Receipt (OR) and Tax Invoice, as required by the Bureau of Internal Revenue (BIR) of the Philippines. The Purchase Order number must be referenced on all invoices.
- The supplier warrants that all equipment (X-ray machine, examination tables, dental kits) shall carry a minimum of two (2) years manufacturer warranty, with service and technical support available within Metro Manila, Philippines. Spare parts and consumables must be sourced locally within the Philippines Manila area to ensure timely maintenance.
- This Purchase Order is governed by the laws of the Republic of the Philippines. Any disputes arising from this Purchase Order shall be resolved through the courts of competent jurisdiction in Metro Manila, Philippines. Both parties agree to attempt mediation before resorting to litigation.
- The Veterinarian practice reserves the right to modify, cancel, or amend this Purchase Order in writing prior to the supplier's confirmation of order acceptance. No verbal modifications shall be binding.
- All data and records related to this Purchase Order shall be retained for a minimum of five (5) years in compliance with Philippine tax and veterinary regulatory requirements.
