1. Acceptance of Purchase Order: By signing below, the vendor acknowledges and accepts all terms, quantities, and pricing specified in this Purchase Order. This document constitutes a binding agreement between Golden Gate Veterinary Clinic and Bay Area Veterinary Supply Co. for the delivery of the items listed herein.
2. Delivery: All items must be delivered to the clinic address listed above in United States San Francisco no later than the Required Delivery Date. The Veterinarian on duty, Dr. Chen or her designated associate, will inspect all shipments upon arrival. Any damaged, expired, or non-conforming items will be rejected and must be replaced within five (5) business days at the vendor's expense.
3. Payment: Payment of the full Purchase Order amount of $25,063.19 USD shall be made via ACH transfer within thirty (30) calendar days of the date of invoice receipt, provided all goods have been accepted in satisfactory condition. Late payments shall accrue interest at 1.5% per month.
4. Regulatory Compliance: All pharmaceutical and biological products must comply with the U.S. Food and Drug Administration (FDA) regulations, the California Department of Food and Agriculture (CDFA) veterinary drug storage requirements, and the California Board of Veterinary Medicine standards.
