- This Quotation Estimate is valid for thirty (30) calendar days from the date of issuance. Any acceptance beyond this period is subject to price revision based on current market rates in Philippines Manila.
- All services shall be performed by a duly licensed Biomedical Engineer registered with the Professional Regulation Commission (PRC) of the Philippines and holding a valid Electronics Engineering license.
- Payment terms: Fifty percent (50%) down payment upon acceptance of this Quotation Estimate; fifty percent (50%) balance upon completion and acceptance of all deliverables. Payment shall be made via bank transfer to the account specified in the official invoice.
- All calibration certificates and compliance reports shall be issued within seven (7) business days following the completion of on-site work in Philippines Manila.
- MediTech Biomedical Solutions Inc. shall provide a twelve (12)-month warranty on all calibration and maintenance work performed by its Biomedical Engineer team, excluding damage caused by misuse, power surges, or unauthorized third-party repairs.
- The client shall provide safe access to all equipment areas, adequate workspace, and necessary utilities (power, compressed air) during the scheduled maintenance windows.
- Any additional services or equipment not listed in this Quotation Estimate shall be subject to a separate written amendment and additional cost approval.
- This Quotation Estimate does not constitute a binding contract until a formal Service Agreement is executed by both parties.
- All work shall comply with the Philippine Electrical Code, DOH Administrative Orders, and applicable IEC standards for medical electrical equipment.
By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms and conditions stated herein. The undersigned Biomedical Engineer confirms that all quoted services meet the professional standards required for medical equipment maintenance in Philippines Manila.
