- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Upon expiry, Brisbane Chemist & Pharmacy Services Pty Ltd reserves the right to revise pricing due to market fluctuations in the Australia Brisbane pharmaceutical supply chain.
- All services rendered by our registered chemist and pharmacy team are governed by the Pharmacy Board of Australia (PBA) standards and the Queensland Pharmacy Act 2010. The chemist responsible for this contract holds AHPRA registration number 1048293 and is a member of the Australian Society of Health-System Pharmacists (ASHP).
- Payment terms: 50% deposit upon acceptance of this Quotation Estimate, with the remaining 50% payable within fourteen (14) business days of the completion of the contracted service period. Invoices will be issued in accordance with Australian Taxation Office (ATO) requirements.
- All PBS-listed medications will be dispensed in compliance with the current Pharmaceutical Benefits Scheme schedule. The client acknowledges that PBS co-payments are the responsibility of the end patient and are not included in this Quotation Estimate unless otherwise specified.
- Brisbane Chemist & Pharmacy Services Pty Ltd maintains a minimum of $20 million in professional indemnity insurance and $10 million in public liability insurance, as required for all chemist operations in Australia Brisbane.
- Any variation to the scope of services outlined in this Quotation Estimate must be agreed upon in writing by both parties. The chemist will provide a supplementary quotation for any additional services requested beyond the original scope.
- This Quotation Estimate does not constitute a binding contract until formally accepted in writing by the client and countersigned by an authorised representative of Brisbane Chemist & Pharmacy Services Pty Ltd.
- All pharmaceutical products supplied will be stored and transported in accordance with Good Distribution Practice (GDP) standards as mandated by the TGA for all chemist facilities operating in Queensland, Australia.
- Disputes arising from this Quotation Estimate or the subsequent service agreement shall be resolved in accordance with the laws of the State of Queensland, Australia, and the jurisdiction of the Brisbane Magistrates Court.
