- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Any acceptance beyond this period is subject to re-confirmation of pricing by KL Wellness Chemist Sdn. Bhd.
- All pharmaceutical products and chemist services listed herein are subject to availability and must comply with the current regulations of the National Pharmaceutical Regulatory Agency (NPRA), Ministry of Health Malaysia, and the Pharmacy Board of Malaysia.
- Payment shall be made within thirty (30) days from the date of invoice issuance. Late payments shall attract a penalty of 2% per month as per Malaysian commercial practice.
- All deliveries within Kuala Lumpur and the surrounding Selangor region shall be made via temperature-controlled vehicles. Delivery times are estimated at 24–48 hours from order confirmation.
- The licensed chemist assigned to this contract shall hold a valid Practising Certificate issued by the Pharmacy Board of Malaysia and shall be available for on-site consultations at the client's Kuala Lumpur facility during agreed business hours.
- Any changes in scope, additional chemist services, or modifications to the product list must be approved in writing by both parties prior to execution. A supplementary quotation estimate will be issued for any such changes.
- This quotation estimate does not constitute a binding contract until a formal Purchase Order or Service Agreement is executed by both parties.
- All prices are quoted in Malaysian Ringgit (MYR) and are inclusive of all applicable taxes unless otherwise stated. Import duties, if any, for foreign-sourced pharmaceutical products shall be borne by the client.
- KL Wellness Chemist Sdn. Bhd. reserves the right to substitute any product with an equivalent NPRA-registered alternative should the original item become unavailable, provided prior written notice is given to the client.
- Confidentiality of all patient data, clinical records, and proprietary information shared during the course of this engagement is strictly maintained in accordance with the Personal Data Protection Act 2010 (PDPA) of Malaysia.
By signing below, the client acknowledges and accepts the terms, conditions, and pricing outlined in this Quotation Estimate for the chemist services and pharmaceutical supply to be delivered in Kuala Lumpur, Malaysia.
