- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. After expiry, all pricing for chemist services and pharmaceutical products may be subject to revision based on market conditions in Thailand Bangkok.
- All prices are quoted in Thai Baht (THB) and are inclusive of applicable Thai VAT at the standard rate of 7%, unless otherwise stated. The VAT registration number of Siam Wellness Chemist Co., Ltd. is 0-1-0105-56789-10-2.
- Payment terms: 50% advance deposit upon acceptance of this Quotation Estimate; the remaining 50% shall be settled within fourteen (14) days of service completion or product delivery in Thailand Bangkok.
- All pharmaceutical products and compounded formulations supplied under this estimate are manufactured, handled, and dispensed by licensed Thai-registered chemists in accordance with Thai FDA standards and GMP (Good Manufacturing Practice) requirements.
- Delivery of all goods and on-site chemist services shall be conducted within the Thailand Bangkok metropolitan area (including Bangkok, Nonthaburi, Pathum Thani, and Samut Prakan). Travel to locations outside this zone will be billed at an additional rate of THB 350 per kilometer.
- Any changes to the scope of work, additional chemist consultations, or extra pharmaceutical items beyond those listed in this Quotation Estimate must be approved in writing by both parties before execution.
- This Quotation Estimate does not constitute a binding contract until a formal Service Agreement or Purchase Order is executed by both parties. Acceptance may be indicated by signing the acknowledgment section below or by written email confirmation.
- Confidentiality: All client data, health records, and proprietary formulation details shared during the engagement in Thailand Bangkok shall be treated as strictly confidential and protected under Thai data privacy law (PDPA 2019).
- Force Majeure: Neither party shall be liable for delays caused by natural disasters, government mandates, or public health emergencies affecting operations in Thailand Bangkok.
