Terms & Conditions of This Quotation Estimate
- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. After the expiry date, prices may be revised based on current market rates for dental materials and consumables in Bangladesh.
- Payment Terms: A 50% advance payment (BDT 31,470) is required to schedule the first treatment appointment. The remaining 50% balance is due upon completion of all procedures. Payment may be made via cash, bKash, Nagad, bank transfer, or credit/debit card at our Dhaka clinic.
- All dental procedures will be performed by qualified and licensed dentists registered with the Bangladesh Medical and Dental Council (BMDC). The lead dentist for this treatment plan is Dr. Farhan Ahmed (BMDC Reg. No: 12345).
- The zirconia crown (Item 4) requires a minimum of 7–10 working days for laboratory fabrication at our in-house dental lab in Mirpur, Dhaka. The patient will be contacted for the fitting appointment once the crown is ready.
- In the event of unforeseen complications during treatment (e.g., additional canals discovered during RCT, bone removal during extraction), a supplementary Quotation Estimate will be issued and must be approved in writing by the patient before any additional work commences.
- This Quotation Estimate does not include emergency dental visits, hospitalization, or general anesthesia. Should general anesthesia be medically required, an additional fee of BDT 15,000–25,000 will apply, subject to a separate written estimate.
- All materials used (zirconia, composite resin, gutta-percha, etc.) are imported from certified international suppliers and carry a manufacturer warranty. Our clinic provides a 12-month warranty on all restorative work performed.
- This Quotation Estimate is non-transferable and is issued exclusively for the named patient. Any change in patient identity will require a new Quotation Estimate.
