This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Payment is due within fourteen (14) days of acceptance of this Quotation Estimate. We accept all major credit cards (Visa, Mastercard, American Express), bank transfer (EFT), and direct debit. A 50% deposit is required to secure your initial Dietitian consultation appointment. The remaining balance is payable upon completion of the final session. Please note that all prices listed in this Quotation Estimate are in Australian Dollars (AUD) and include Goods and Services Tax (GST) at the current rate of 10% as applicable in Australia Brisbane and across Queensland.
- This Quotation Estimate is a good-faith estimate of costs and is not a fixed-price contract. Final charges may vary slightly depending on the complexity of the dietary intervention required by the Dietitian.
- All Dietitian services are provided in accordance with the National Competency Standards for Dietitians in Australia and the Code of Ethics of the Dietitians Association of Australia.
- Cancellations must be made at least 24 hours in advance. Late cancellations or no-shows will incur a fee of 50% of the session cost.
- Our Dietitian practice operates Monday to Friday, 8:00 AM to 5:00 PM (AEST), in Australia Brisbane. After-hours emergency dietary advice is available by telephone for a surcharge of $50.00 per call.
- Client health information is handled in strict confidence in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.
- This Quotation Estimate does not constitute a medical diagnosis. The Dietitian will work in conjunction with your General Practitioner or treating specialist where appropriate.
- Any additional services not included in this Quotation Estimate will be quoted separately and require written client approval before commencement.
- Disputes arising from this Quotation Estimate shall be resolved in accordance with the laws of the State of Queensland, Australia.
