Payment Terms

A 30% deposit of $645.15 (inclusive of GST) is required to secure your booking and commence the Dietitian programme.
The remaining balance of $1,505.35 is payable in two equal instalments: 50% at the midpoint of the programme (Week 6) and 50% upon completion (Week 12).
All payments are to be made via bank transfer, credit card, or EFTPOS at our Australia Sydney clinic. Cheques are not accepted.
Payment is due within 14 days of invoice date. Late payments may incur a 2% monthly interest charge in accordance with Australian commercial law.
Health Fund rebates: This Quotation Estimate is eligible for partial or full rebate through most major Australian health insurance providers. Please confirm your Dietitian service coverage with your health fund prior to commencing treatment.

Terms and Conditions

This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Pricing is subject to change after the validity period expires.
All Dietitian services are delivered by Ms. Sarah Mitchell, an Accredited Practising Dietitian (APD) registered with the Dietitians Association of Australia and the Australian Health Practitioner Regulation Agency (AHPRA).
Cancellations must be provided with a minimum of 48 hours' notice. Late cancellations or no-shows will be charged at 50% of the session fee.
This Quotation Estimate does not constitute a medical diagnosis or treatment plan. All dietary recommendations are to be used in conjunction with your primary healthcare provider's guidance.
Our clinic operates in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles. All client information is stored securely and in compliance with Australian data protection legislation.
Disputes arising from this Quotation Estimate shall be resolved in accordance with the Australian Consumer Law and the jurisdiction of the State of New South Wales, Australia Sydney.
This Quotation Estimate is non-transferable and is prepared exclusively for the named client.

Acceptance of Quotation Estimate: By signing below, the client acknowledges and accepts this Quotation Estimate for the Dietitian services described herein. The client confirms that they have read, understood, and agree to all terms and conditions outlined in this document.
