1. This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, pricing for the Dietitian services listed herein may be subject to revision based on market conditions in Bangladesh Dhaka.
2. All Dietitian consultations and services described in this Quotation Estimate will be conducted by Dietitian professionals registered with the Bangladesh Dietetic Association (BDA) and holding valid practice licenses issued by the relevant regulatory authority in Bangladesh Dhaka.
3. Payment terms: 50% advance payment upon acceptance of this Quotation Estimate, and the remaining 50% due upon completion of the service period. Payments may be made via bank transfer, bKash, Nagad, or cash at our Bangladesh Dhaka office.
4. The client agrees to provide accurate medical history, current medication lists, and relevant laboratory reports to the assigned Dietitian prior to the initial assessment session.
5. Cancellation policy: Cancellations made more than 48 hours before a scheduled Dietitian session will be rescheduled at no additional cost. Cancellations within 48 hours or no-shows will incur a 50% fee of the session rate.
6. All nutritional plans and Dietitian recommendations provided under this Quotation Estimate are for guidance purposes and do not replace medical treatment prescribed by a licensed physician. In case of medical emergencies, the client must seek immediate hospital care in Bangladesh Dhaka.
7. This Quotation Estimate does not include the cost of laboratory tests, diagnostic imaging, or prescription medications. These will be billed separately at the respective facility rates in Bangladesh Dhaka.
8. NutriCare Bangladesh reserves the right to reassign a different Dietitian professional if the originally assigned Dietitian is unavailable, provided the replacement holds equivalent or superior qualifications.
9. All data collected during Dietitian consultations will be kept strictly confidential in accordance with the Digital Security Act of Bangladesh and our internal data protection policy.
10. Any disputes arising from this Quotation Estimate shall be resolved through amicable negotiation. If unresolved, the matter shall be subject to the jurisdiction of the courts in Bangladesh Dhaka.
