The first installment is due upon acceptance of this Quotation Estimate.
• Scheduling: All Dietitian consultation sessions must be scheduled at least 48 hours in advance. Rescheduling or cancellation with less than 24 hours' notice will incur a fee of 50% of the session value.
• Confidentiality: The Dietitian and all staff of Verde Vida Nutrição & Saúde Ltda. are bound by the Brazilian General Data Protection Law (LGPD – Lei Geral de Proteção de Dados, Law No. 13.709/2018) and the professional ethics code of the CFN. All client health information, dietary records, and consultation notes will be kept strictly confidential and stored in compliance with applicable Brazilian data protection regulations.
• Scope Limitation: The Dietitian services described in this Quotation Estimate are strictly nutritional and do not constitute medical diagnosis, prescription of pharmaceutical drugs, or replacement of care provided by a physician. In cases where a medical condition requires joint management, the Dietitian will coordinate with the client's attending physician in Brasília.
• Location: All in-person services will be performed at the Dietitian's registered office in the Asa Sul district of Brasília, DF, or at the client's designated location within the Federal District, as agreed upon in writing. Travel outside the Distrito Federal will be quoted separately.
