This Quotation Estimate is formally issued by NutriVida Dietetics & Wellness Center, a licensed and registered Dietitian practice operating in Brazil São Paulo, to provide the client with a comprehensive, transparent, and itemized financial projection for all professional dietetic services to be rendered. This Quotation Estimate for Dietitian services in Brazil São Paulo is subject to the following payment conditions:
- Payment Schedule: The total amount of R$ 6,440.00 may be settled in a single payment or divided into up to 6 (six) monthly installments of R$ 1,073.33, with the first installment due upon acceptance of this Quotation Estimate.
- Accepted Payment Methods: Bank transfer (TED/PIX), credit card (up to 3 installments), or debit card. All transactions are processed in Brazilian Reais (BRL) in compliance with local financial regulations in São Paulo, Brazil.
- Invoice Issuance: A formal Nota Fiscal (NF-e) will be issued for each payment in accordance with the tax regulations of the State of São Paulo and the Brazilian Federal Revenue Service (Receita Federal).
- Late Payment: Overdue installments will be subject to a 2% fixed penalty plus interest calculated at the Selic rate, as per Brazilian civil law.
This Quotation Estimate is valid for a period of 30 calendar days from the date of issue. After this period, prices for the Dietitian services listed herein may be subject to revision based on market conditions, operational costs, or regulatory changes in Brazil São Paulo. The client's acceptance of this Quotation Estimate constitutes a binding agreement for the delivery of the specified dietetic services.
Important Note: All Dietitian services described in this Quotation Estimate are provided by professionals registered with the CRN-3 (Conselho Regional de Nutricionistas – 3ª Região), which governs the practice of dietetics in the state of São Paulo, Brazil. No medical prescriptions are issued; all dietary recommendations are within the legal scope of the Dietitian profession as defined by Law No. 8,234/1991 of the Federative Republic of Brazil.In the event the client cancels the engagement prior to the commencement of Dietitian services, no fees shall be charged. If cancellation occurs mid-program, a pro-rata refund will be calculated based on sessions already completed. Cancellations must be communicated in writing at least 48 hours in advance. This policy is consistent with consumer rights protections under Brazilian law applicable in Brazil São Paulo.
All personal health information, dietary records, and clinical data shared during Dietitian consultations are protected under the Brazilian General Data Protection Law (LGPD – Lei Geral de Proteção de Dados, Law No. 13,709/2018).
