- Validity: This Quotation Estimate is valid for thirty (30) days from the date of issue. After the expiration date, pricing may be subject to revision based on current market rates for Dietitian services in Vancouver, Canada.
- Payment Terms: A 50% deposit is required to secure the initial consultation appointment. The remaining balance is due within fourteen (14) days of service completion. Payment may be made via Interac e-Transfer, credit card (Visa, Mastercard, Amex), or certified cheque. All amounts are in Canadian Dollars (CAD) and are subject to applicable Goods and Services Tax (GST) at 5% and Harmonized Sales Tax (HST) where applicable in British Columbia.
- Cancellation Policy: Appointments cancelled with less than 48 hours' notice will be subject to a 50% fee of the scheduled session rate. No-shows will be charged the full session fee. This policy applies to all Dietitian consultations conducted in the Vancouver, Canada area.
- Confidentiality: All client information, medical records, and dietary data shared during consultations are protected under the Personal Information Protection Act (PIPA) of British Columbia and the professional code of ethics of the College of Dietitians of BC. The Dietitian will not disclose any client information without written consent.
- Scope Limitation: The services outlined in this Quotation Estimate are for nutritional counselling and dietary management only. The Dietitian does not provide medical diagnosis, prescribe medication, or replace the services of a licensed physician. Any medical conditions identified during assessment will be referred to the client's primary care provider in Vancouver, Canada.
- Revisions: The meal plan and dietary recommendations may be revised up to two times at no additional charge during the engagement period. Additional revisions will be billed at the standard hourly rate of $110.00 CAD.
- Acceptance: By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms and conditions outlined herein. The engagement will commence upon receipt of the initial deposit.
